Pdf does not display correctly in preview

We've tried several makes and models of printers. No dice there. We've tried exporting as a PDF and printing from several other applications. All of them work and print it fine except for Word Even previous versions of word will print it fine. I've removed most of the personal info on the form but left a date of birth so you can see how it reacts to the text boxes as well. EMF is a vector format, but there are no vectors in the art, just scans of pages.

EMF was the wrong choice of format for this application. While an EMF should maintain it's size when printed, I'm guessing that the fact that they contain only bitmaps is causing them to behave in a non-standard way. That's what I feared. Odd though, I guess I just assumed it was a PNG since its the default option when you right click the image and save it.
