I am not keen on reinstalling Vista Ultimate as I am afraid that the problem will return since VU is up and running for just two weeks now. The fix in my case was a little more involved. The UAC was corrupted by earlier attempts to get a domain user to login and have their profile saved when they log off. After working with Microsoft for six plus hours and three different techs, the Windows was so corrupted that I had to delete the partition and start from scratch.
After the clean install, I still ran into the issue that was the original problem. I was login in as a domain user and the user profile was recreated every time that I logged in either of two domain users.
This did not happen to the Domain Administrator. On the third call to Microsoft, we brought in an ADS specialist and in ten minutes, he identified the original problem. The users were members of the Domain Guest user group.
This does not affect the users running Windows XP, but a Vista user is then locked into a mandatory profile. So any changes that a user makes are not saved when they log out. Simply removing the users from the group and running a gpupdate corrected the problem. It was such a simple resolution, but what a very long and painful path to get there. Had you upgraded to Internet Explorer 6 Service Pack 1 yet? I have been doing windows update until about a month ago but I do not recall having upgraded to Internet Explorer 6 Service Pack 1.
I was hoping you could avoid reinstalling Internet Explorer with your Firewall and and anti-virus programs running, but it looks like we'll have to try it.
